Responsibilities include:

• Providing high-quality patient care to stabilize and monitor patients post-acute or surgical procedures
• Monitoring vital signs, administering medications, and managing complex patient needs
• Collaborating with interdisciplinary teams to develop and implement care plans
• Documenting patient progress accurately and efficiently
• Educating patients and families on post-discharge care
